The Medical University of South Carolina, Department of Medicine, in accordance with South Carolina Procurement Code, Section 11-35-1560 and regulation 19-445.2105, is providing public notice of the Intent to Sole Source a contract to Fora Care Inc, for a five (5) year renewal terms. The annual value of the contract is $135,000.00.
The Department of General Internal Medicine is requesting to purchase diabetes supplies from Fora Care Inc. The requested supplies is for an on-going research project-TACM-2 Telehealth-- for diabetics in the rural South Carolina areas. Any deviation from this product/monitoring service would cause extreme disruption to our remote patient monitoring intervention which encompasses multiple geographic areas in South Carolina. Currently there are 6 Professional Service Agreements for the research project (PL18271094-Shifa Free Clinic; PL18275656-Dillon County Free Medical Clinic; PL18280005-Helping Hands Free Medical Clinic; PC19280006-Little River Medical Center, Inc.; PA19281120-Regenesis Organization & PI19308784-St. James-Santee Family Health Center). The purpose of this research program is to help low income patients achieve and maintain better self-management skills and improve blood sugar levels, using the FORA 2-in-1 Telehealth system for diabetes management intervention. This achieve by linking a case manager to patients with poorly controlled type 2 diabetes in real time. Medications are titrated under supervision of primary care and/or endocrinology physicians. Fora Care, Inc. is the sole manufacture and distributor of the FORA 2-in-1 Relehealth System.
PROTESTS (MAY 2019): If you are aggrieved in connection with the intended award of this contract, you may be entitled to protest, but only as provided in §11-35-4210. To protest an intended award of a contract pursuant to §11-35-1560, you shall (i) notify the chief procurement officer in writing of your intent to protest within five (5) business days of the date this intent to award is posted, and (ii) if the appropriate chief procurement officer has been timely notified of your intent to protest, you may submit your actual protest within fifteen days of the date this notice of intent to award is posted. Days are calculated as provided in §11-35-310(13). Both protests and notices of intent to protest must be in writing and must be received by the appropriate Chief Procurement Officer within the time provided. The grounds of the protest and the relief requested must be set forth with enough particularity to give notice of the issues to be decided.
